The Transforming Communities Initiative (TCI) is an innovative funding and technical assistance initiative, partnering with Trinity Health, its Regional Health Ministries (RHMs), such as Saint Agnes Medical Center, community-based organizations and community residents to advance health and racial equity in communities experiencing high poverty and other vulnerabilities. Together with Trinity Health, community partners, through a multi-sector collaborative, work to develop and implement evidence-based strategies that advances health and racial equity through addressing the intersection of community health, safety and crime – one of several root causes of poor health outcomes identified in the most recent Community Health Needs Assessment (CHNA) Implementation Strategy.

Southwest Fresno POWER is a collaborative of southwest Fresno serving community-based organizations, seeking to ensure a healthier and safer southwest Fresno.

The SWF POWER initiative aligns with TCE’s overarching mission to foster power infrastructure and build resilient communities. The California Endowment has a strong commitment to investing in community organizing and grassroots efforts that are led by and for the people most affected by inequities. Their support for our work demonstrates a shared belief that building power and agency in communities is essential for dismantling racist systems and creating lasting, positive change.

By funding initiatives like SWF POWER, TCE helps to:

  • Strengthen the capacity of local organizations to drive transformative change.
  • Develop a “network of power-building collaboratives” within communities like southwest Fresno.
  • Address the root causes of poor health and injustice in communities.
  • Invest in leaders who are closest to the issues, ensuring that solutions are community-driven and effective.
