The Transforming Communities Initiative (TCI) is an innovative funding and technical assistance initiative, partnering with Trinity Health, its Regional Health Ministries (RHMs), such as Saint Agnes Medical Center, community-based organizations and community residents to advance health and racial equity in communities experiencing high poverty and other vulnerabilities. Together with Trinity Health, community partners, through a multi-sector collaborative, work to develop and implement evidence-based strategies that advances health and racial equity through addressing the intersection of community health, safety and crime – one of several root causes of poor health outcomes identified in the most recent Community Health Needs Assessment (CHNA) Implementation Strategy.<\/p>
